Care Management Entity (CBAY)

Services

The Care Management Entity (CME) serves youth throughout Georgia with significant behavioral health needs who are at risk of out of home placements. The CME is a partnership with the Department of Behavioral Health and Developmental Disabilities' Community Based Alternatives for Youth (CBAY) program.  View Point Health provides care through two core strategies:

Intensive Customized Care Coordination: A strength-based, family-driven approach focused on developing self-sufficiency; building natural supports; and increasing family capacity to respond to crises. The evidence-based practice model used is called High Fidelity Wraparound, which is a team-based process, demonstrating outcomes in reducing restrictive placements; improving functioning in school, home and community so that youth can thrive in their community.

System of Care Coordination: Helps child-serving agency staff identify local, high quality behavioral health services and additional supports to meet the unique needs of youth and families.

Our System of Care Coordinators also serve as a single point of entry for Georgia's System of Care, and help partners and families get the appropriate services they need. View Point Health is continually developing a network to increase access of quality services and supports. This service is available for all youth with complex needs, whether eligible or not for Intensive Customized Care Coordination.
