Intensive Case Management


Description of services:
ICM encompasses those clients diagnosed with a Severe & Persistent Mental Illness who do not meet ACT criteria but still exhibit a need for intense services due to multiple incarceration, hospitalizations, & ongoing bouts of homelessness.

ICM services consist of 2 teams which cover the 5 counties of Gwinnett, Rockdale, & Newton & then DeKalb & Fulton.

Clients will be visited once a week in their homes or within their communities by an assigned Case Manager who will assist clients in locating, accessing, & navigating community resources.  Case Managers will also coach clients in the development of those daily living skills needed toward achieving maximum health and potential.

ICM services will include on-call staff to provide support and crisis management 24/7.

ICM program philosophy:
The philosophy of ICM services is to assist individuals in reaching their optimum level of wellness and functional capacity through advocacy, education, communication, identification of service resources and service facilitation resulting in less incarcerations, hospitalizations, and episodes of homelessness.

Making a referral for ICM services:
Referrals will be accepted interagency & accepted from community partners who service clients in these 5 counties. 

Review Admissions Criteria attached to the referral form, complete the referral leaving no entry blank to avoid delay of processing, & submit via email listed on the bottom of the referral form.
